Well-Child & Sports Physicals for all FUSD Students
Appointments start on August 26, 2024
- Parent/Legal guardian must accompany the student
- Bring current immunization record
- Must bring Medi-Cal card if your child has Medi-Cal insurance card
Sports Physicals: Please wear comfortable clothes and shoes.

PRIVATE INSURED STUDENTS, please call your healthcare provider for immunizations or you can call CVS, Rite-Aid, Walgreens or Walmart pharmacies to see if they take your insurance and offer the immunizations your child needs.
