Required Documents for Infant, Toddler and Preschool Enrollment
Below is a list of documents required to complete your child’s application.
1. Birth Certificate Verification for the enrolling student.
2. Birth Certificate Verification for all siblings under the age of 18 still living at home.
3. Current Proof of Income dated within 60 days of your application date.
If you get paid:
- Weekly: four paystubs.
- Every two weeks: two paystubs (pay cycle is usually 14 days).
- Twice a month: two paystubs (pay cycle is usually 15 days or longer – pay received 1st & 15th or 5th & 20th).
- Monthly: pay is received once per month.
4. Current Proof of Address dated within 60 days of your application date. One of the following: PG&E, Telephone-landline, Utility or Rental Agreement with current rent receipt.
5. Health Physical including TB Results given within a year of entry into school.
6. Immunization Record for the enrolling student. The California School Immunization Law requires that children be up-to-date on their immunizations, as shown in the table below:
Why Do We Collect Income Information?
We collect income information to determine if your family qualifies for state-funded programs like the California State Preschool Program (CSPP) and the General Child Care and Development Program (CCTR). Your income helps us confirm eligibility based on state guidelines which we are required to follow.
All information is kept confidential and is only used to comply with state regulations set by the California Department of Education and the California Department of Social Services.
For the 2025-2026 school year, children born between December 1, 2020, and September 1, 2021, are eligible for enrollment in either the TK or Preschool Program.
Completing the Early Learning application does not guarantee enrollment in your preferred school or class. We will notify you by phone regarding your child’s enrollment and placement. We look forward to welcoming your little one!

3. Health Evaluation by a health care provider (health physical)/ Evaluación de Salud por un proveedor de atención médica (examen físico de salud)
Please completely filled out by the child's doctor. Please make sure the box titled "Screening of TB Risk Factors" is completed by the doctor.

5. Self-Certification of Income/ Autocertificación de Ingresos
When no other documentation is available, this form is used to document income. Please record undocumented employment income (if no check stubs are available), non-employment income, periods of zero income or self-declared income for a means-tested governmental program when the application is not available.

6. Statement of Earning/ Declaración de Ganancias
This form is to be used for employees receiving salary, on leave or vacation (during any period), fluctuating pay and new hires. All sections must be filled out and must be signed by the employer.
