Title VI Indian Education 506 Form
Eligibility of Students:
All American Indian and Alaskan Native students enrolled in the school district are eligible to participate in the Fresno Unified's American Indian Education Program by completing the Title VI Student Eligibility Certification, 506 Form. What is a 506 form?
The 506 form is an official, federal document from the Office of Indian Education used to collect federal funding for our programs. Please fill out the 506 form ED_506_form_2026.pdf.
Eligible Students include those who are:
- A member of an Indian Tribe or band, as membership is defined by the tribe or band, (including any tribe or band terminated since 1940; and any tribe or band recognized by the state in which the tribe or band resides).
- A descendant in the first or second degree (parent or grandparent as described).
- Considered by the Secretary of the Interior to be an Indian, Eskimo, or Aleut.
- Member of an organized Indian group that received a grant under the Indian Education Act of 1988 as in effect the day preceding the date of enactment of the Improving America’s Schools Act of 1994.
Why should I fill out a 506 form?
Every completed 506 form helps support the American Indian Education Program in meeting the unique academic and cultural needs of American Indian/Alaskan Native students in the district. Each year the program's funding is determinded by the number of valid 506 froms submitted.
